Your Role as a Transition Coordinator:

  • Develop and Implement Career & Transition Services – Create individualized transition plans for students, coordinate job placements, arrange transportation, and monitor student progress.
  • Student Instruction & Coaching – Teach students career readiness skills, including resume writing, interviewing techniques, workplace behavior, and independent living skills.
  • Career Exploration & Readiness – Facilitate hands-on experiences, including in-school skill development programs and community-based job placements, to help students transition into work and adulthood.
  • Collaboration & Relationship Building – Work closely with teachers, families, and community organizations to identify and create vocational opportunities for students.
  • IEP Compliance & Documentation – Maintain accurate records of student progress, ensure deadlines are met for career and transition goals, and provide required reports for Individualized Education Plans (IEPs).
  • Community Engagement – Build relationships with local businesses and organizations to expand job placement opportunities for students.
  • Safe Student Support – Utilize non-violent crisis intervention techniques, assist in de-escalation when necessary, and provide physical guidance to students as needed.
  • Transportation & Supervision – Transport students to work sites or career readiness activities in a company vehicle as needed.

What Helps You Thrive in This Role:

  • You have a bachelor’s degree in education, social work, or a related field
    (A master’s degree is even better!)

  • You bring at least one year of experience supporting students with special needs in a school setting, and another year helping with transition services to set them up for success.

  • You’re confident in your ability to build strong relationships and communicate clearly with students, families, school staff, and community partners.

  • You understand Pennsylvania Department of Education regulations around special education and transition services, and know how to apply them.

  • You’re familiar with child development, behavioral disorders, and the basics of Applied Behavior Analysis (ABA), and you use that insight to guide your approach.

  • You’re organized, manage your time well, and show up with a professional attitude.

  • You hold or are eligible for a PA Private Academic Certification or Public Special Education Certification, which helps you support students even more effectively.
