Your Role as a Transition Specialist:

  • Provide personalized vocational and real-life situational instruction to students.
  • Manage artifacts according to Career Readiness and Chapter 339 requirements.
  • Develop, implement, and regularly revise Individual Transition Plan (ITP) goals in collaboration with the IEP team.
  • Facilitate transition programs, including career skills assessments, exploration activities, and instruction on independent living.
  • Coordinate school and community-based learning opportunities.
  • Network with community agencies and organizations.
  • Collaborate with Transition Workers, Coordinators, and multidisciplinary team members.
  • Monitor student job performance and progress for report card development.
  • Conduct ongoing assessments to determine student transition needs.
  • Create materials for individual and group transition sessions.
  • Document student sessions and maintain daily logs in alignment with IEP guidelines.
  • Attend multidisciplinary team meetings, IEP meetings, and other required meetings.
  • Complete various reports as directed.
  • Travel to various school locations as necessary.
  • Attend and complete all required training and supervision sessions.
  • Transport students in a company vehicle as needed for program activities.
  • Use safe crisis management techniques.
  • Adhere to safety practices, the Exposure Control Plan, and wear Personal Protective Equipment as required.
  • Maintain student confidentiality according to FERPA and company policies.
  • Perform other related duties as assigned.

Required Qualifications:

  • Bachelor’s degree in education or related field.
  • Minimum two years of experience providing transition services to students with severe disabilities.
  • Valid PA Driver’s License.
  • Ability to drive a company van and pass a defensive driving course.
  • Strong interpersonal skills for effective relationships with students, parents/guardians, school personnel, and community partners.
  • Knowledge of common behavioral disorders in children and adolescents.
  • Ability to manage defiant or non-cooperative students with consistency and firmness.
  • Skilled in crisis intervention techniques for managing physically aggressive students.
  • Quick and agile enough to intervene in situations where students may harm themselves or others.
  • Capable of physically assisting in student evacuations during emergencies.
  • Professional appearance and demeanor.

Preferred (Nice to Have) Qualifications:

  • Master’s degree preferred.
  • PA Private Academic Certification or Public Special Education Certification preferred.
